Minimally invasive techniques allows surgery to be done through smaller incisions to cause less collateral damage to the spine and surrounding soft tissues compared to traditional open surgery.  With this approach, rather than cutting through and stripping the muscles off the spine, specialized instruments are used to gently spread the muscles and allow surgery.

Most commonly in MISS, a small incision is made and a tubular retractor or portal is inserted into it. This device pushes aside all the soft tissue, giving the surgeon access to the spine. Then, small tools are inserted through the tubular retractor one at a time to perform the necessary steps required during the surgery. 

Multiple incisions and retractors are sometimes necessary to treat a complicated spinal issue. The entire procedure is often done with the aid of a loope magnification, microscope and/or fluroscopy, because the tools and working area is so small.
